Submitting an Individual MU-4 Form

Procedure

  1. Login to NMLS.
  2. Click the Filing tab.
  3. Select Individual from the menu.
    The Individual Filings screen is displayed.
  4. Click the Request New/Update button.
    Note: If you do not see the Request New/Update button, you may already have a pending filing and will need to select the Edit button to continue editing your pending MU4 filing or the Delete button to delete the pending filing that is outstanding.
  5. Click Continue to proceed with the filing.
    Any existing license filings will appear.
  6. Warning: Before you proceed, note: NMLS will allow you to select all states, even if your company or branch is not licensed in those states. Verify which state licenses you need. FEES ARE NON-REFUNDABLE
    Click Add to select additional licenses.
    The list of states appears in the License Wizard.
  7. Select only the states you have verified with your company and click Next.
    The Existing License Registrants screen is displayed.
  8. If you are transitioning an existing LO license, enter your current LO license number (not the company number) in Existing License/Registration Number. If you do not have a current license, leave Existing License/Registration Number blank. Click Next.
    If you entered an existing license, the license for transition is displayed.
  9. Verify the display is correct.
    1. If it is correct, click Finish.
    2. If it is incorrect, click Previous to return to the Existing License Registrants screen.
    The License Registration Information screen is displayed, showing total charges in the top right.
  10. Click Next to continue.
    The Identifying Information screen is displayed.
  11. Enter the requested information in each field and click Save.
  12. Click Next to continue.
  13. Enter any “other names” you have been known by or used since the age of 18 by clicking the Add button. Click Next.
    The Residential History screen is displayed.
  14. Note: Ten years of residential history with NO GAPS is required.
    Click Add to enter details for each location. Repeat as needed.
    Note: The format for dates is MM/YYYY.
    Note: When entering your CURRENT address, leave the To field blank.
  15. Once complete, click Next.
    The Employment History screen is displayed.
  16. Note: Ten years of employment history with NO GAPS is required.
    Click Add to enter details for each period:
    Note: The format for dates is MM/YYYY.
    1. If a student, enter the school name and address.
    2. If unemployed, enter Unemployed in Position Held field and list your home address.
    3. Select Yes or No for Is the employment financial services related?
    4. Click Save.
    5. Repeat as needed.
  17. Once complete, click Next.
    The Other Business screen is displayed.
  18. Click Add to provide details on other business with which you are involved. If none, click Next.
    The Disclosure Questions screen is displayed.
  19. Answer all disclosure questions.

    Any Yes answer will require explanation to be provided in the Disclosure Explanations section. See Individual Disclosure Explanations.

  20. Select Add to provide an explanation if necessary.
